First, download and read the Introduction and Instruction form. It is not necessary to print this one out.
Second, download and print the Naturopathic Wellness Center Intake form.
Third, download and print out 4 of the Diet Diary pages.
All the paperwork (including copies of labs and reports) must be received in plenty of time to be reviewed before the appointment.
Paperwork can be dropped off or mailed to the office.
